How to get a Community Care assessment (if you live in Norfolk,UK).
Phone Norfolk County Council Customer Services on
0344 800 8014. This is a general County Council phone number.
Say you are a disabled parent and you need an assessment from Adult Social Services. It’s really important to say you are a disabled parent because it tells them what to do next.
Customer Services will ask you for your name and contact details, and briefly why you need an assessment.
They will pass your details on to the Social Services Access Service.
OR
Fill in the online contact form Online Social Care Enquiry Form. This goes straight to the Access Service.
Say you need an assessment as a disabled parent. It’s really important to say you are a disabled parent on the online form because it tells them what to do next.
The Access Service will contact you to ask for more information. They will ask about your general needs, then pass your details to your local area Adult Social Services (usually) or Children’s Services.
Someone from your local area may speak to you then, or soon after to arrange what to do next.
Getting help to get an assessment.
Anyone can help you to get an assessment as a disabled parent, or you can ask for one yourself.
You can ask a social worker, doctor, health visitor, Community nurse, midwife, therapist, advisor, friend, supporter or advocate to help you get an assessment of your needs.
If you are unsure, ask someone you trust to be with you.
You can ask for an interpreter, an advocate, a communication assistant or a friend or family member to be with you during assessments.
You should be given information about the assessment in a way you can understand.
